This is the gap almost every small firm has, and it is the one an inspector finds first.

The 1970 regulations were revoked. PUWER now requires demonstrable competence to select, mount or use a wheel. Covers mounting, the 3mm work rest gap, ring testing, guarding and dust.
24 workers were killed by a moving vehicle at work in 2025/26, second only to falls. The marshal is often the one killed, because they stood in the vehicle's path.
31 workers killed in 2025/26, the single biggest cause. Most were short falls from ladders and fragile roofs. There is no minimum height in the regulations.
HSE is running over a thousand inspections and has ordered firms to stop work. Dry cutting must stop. Silicosis is irreversible and often fatal.
The largest cause of lost working days. The regulations say avoid first, then assess, then reduce. A lifting course on its own does not discharge the duty.
PPE comes last in the hierarchy, after elimination, substitution and engineering controls. Reaching for a mask first is the failing inspectors find fastest.
Around 5,000 deaths a year, more than road traffic collisions. The regulations require this training for anyone who could foreseeably disturb it.
A breaker reaches the exposure action value in about five minutes of trigger time. HAVS is permanent nerve and circulatory damage.
A cubic metre of soil weighs over a tonne. People are killed in trenches under 1.5 metres. Inspection required at the start of every shift.
The two big site killers are overhead lines and buried cables. Reach plus 9 metres from wooden poles, plus 15 from steel pylons.
Wet cement is pH 12 to 13, oven cleaner territory. It burns slowly and painlessly, so people carry on working.

Selling to a child carries an unlimited fine. Three sales in three months and the court can suspend your alcohol licence for three months. The defence exists, but only if you can evidence it.
Under the Misuse of Drugs Act it is an offence for management to knowingly permit drug use or supply, and turning a blind eye counts. A licence review can revoke.

These are template and guidance documents. The legal duty stays with the employer, and every pack says so on its face. You must make the risk assessment specific to your own site rather than simply signing it. If your situation is unusual, take advice.

You assess your people in-house using the pack and send us the completed records. We check them and issue a certificate carrying a QR code that resolves to our verification page. A site safety officer scans it and either gets a real record or nothing at all.
If it does not scan, it is not ours.

We verify the evidence properly and we reject records that are too thin. A certificate that means nothing is worth nothing. Certificates state clearly that the assessment was carried out in-house and verified by us.
For plant and machinery, where an outside assessor genuinely carries weight.

An external assessor is not a legal requirement. It is a credibility one: a principal contractor takes an outside assessment seriously and an unqualified in-house one much less so.
